The information provided on this website is intended to describe the counseling and wellness services offered by Brandee Evans, M.A., LPC-Associate, supervised by Ana Collazo, LPC-S. Website content is provided for general informational and educational purposes and should not be considered a substitute for individualized mental health treatment, medical care, diagnosis, or professional advice.

Use of this website, submitting an inquiry, requesting a consultation, or communicating through the website does not automatically establish a therapist-client relationship. A therapeutic relationship is established only after appropriate intake procedures, informed consent, and acceptance for services have been completed.

This website, email, voicemail, online scheduling system, and client portal should not be used for emergency or crisis services.

If you are experiencing a mental health emergency, believe you may harm yourself or someone else, or require immediate assistance, call 911, go to the nearest emergency department, or call/text 988 to reach the Suicide & Crisis Lifeline.

Appointment requests are subject to availability and are not confirmed until you receive confirmation from the practice.

A complimentary 15-minute consultation may be offered to prospective clients to discuss general needs, services, scheduling, and potential therapeutic fit. A consultation is not psychotherapy, a clinical assessment, or emergency/crisis care and does not establish an ongoing therapeutic relationship.

Clients are responsible for understanding the financial policies associated with their care, including applicable session fees, insurance benefits, copayments, coinsurance, deductibles, cancellation fees, and services that may not be covered by insurance.

Acceptance of an insurance plan does not guarantee that a particular service will be covered or that the provider is in-network with every variation of that plan. Clients are encouraged to verify their individual behavioral health benefits directly with their insurance carrier.

Additional administrative, documentation, records, and court/legal services may carry separate fees as outlined in the practice's financial policies.

Appointments canceled with less than 24 hours' notice or missed without notice may be subject to a $75 late cancellation/no-show fee. Cancellation and no-show fees are generally not billable to insurance.

Please refer to your signed financial and informed-consent documents for the complete cancellation policy and any applicable exceptions.

Telehealth services are provided only when clinically appropriate and consistent with applicable licensing requirements. Clients must accurately disclose their physical location at the time of telehealth sessions when requested.

EMDR therapy may be incorporated when clinically appropriate. Participation in EMDR is determined collaboratively and may require assessment, treatment planning, stabilization, preparation, and development of appropriate coping resources before trauma reprocessing begins.

Information about EMDR on this website is educational and does not guarantee that EMDR will be appropriate for every client or condition.

Ember Haven Counseling & Wellness, PLLC takes reasonable measures to protect client privacy and confidential information. However, no method of electronic communication can be guaranteed to be completely secure.

Clients should avoid sending highly sensitive clinical information through ordinary email, social media, or other non-secure communication methods. When available, secure communication methods and the client portal should be used for confidential information.

The practice's Notice of Privacy Practices provides additional information regarding how protected health information may be used, disclosed, and safeguarded. HHS requires covered providers to describe privacy rights and permitted uses and disclosures in that notice. (HHS.gov)

Because you're practicing as an LPC Associate, I would have Ana review the final version before publishing. Texas specifically requires LPC Associates to identify Associate status and their supervisor in counseling advertisements/announcements, and BHEC cautions practitioners to rely on current rules because requirements can change.
